I vote for universal joints or driveshaft. On the jack stands puts about 0 load on the drivetrain, so if it did not click I would guess it is load related instead of a 1st gear only thing. It just happens that 1st gear gives you the highest loads on the drivetrain.
My friend'* Bronco 2 was clicking, really only doing it when starting from a stop, and he lost his driveshaft a couple weeks later. And the harder he accelerated the worse it would click. When the shaft went out he was driving on the highway so he wasn't in 1st gear, but he was going up hill so I'm sure he had it floored in order to maintain his speed.

Sounds like torque converter bolts to me....it'* a common issue with the Blazer'* and the 1500 C/K series Chevy'* of that year area....Pretty much anything RWD/4WD that had the 700R4/4L60E'* at that point.
I had the same exact thing happen to my 94 Chevy 1500 a yr or two ago.
The TQ spec on them is 75 ft/lbs if you get a chance to check the torque.
