You can rule out exhaust. Unless the O2 isn't in the exhaust pipe and there'* a 3" hole... it'* not likely. It takes a lot of air to do this.
Being that I'm a jerk and I'd like to follow a certain path. Can we please go back to the questions I asked. By doing this we can narrow things down and save you a lot of hair pulling. I've had, seen and solved this code many many times. It'* really an easy code to solve in my mind.

While checking techinfo is the best place to start. This code is a fun one. I've mastered it and that'* not an easy thing to say.
Please search my username and P0171 if you want to help me out from typing it all again.
Has the LIM gasket been replaced?
Have any vacuum lines been disturbed?
Has the battery been unhooked or changed?
Once you search on the code, you'll understand better. I'm pretty sure I've written explanations to it all. Heck..I'm off to search it for you. be right back.

The LIM has not been replaced. The vacuum lines have not been disturbed. And I always disconnect the battery cables before working on any of the engine electronics. Hope that answers those questions.

Have you unhooked the battery cables any time recently?
Are they loose if you wiggle them.
I'd suggest pulling both, removing the bolts out of the rubber and pulling the boots off for a detailed inspection.
My first acquaintence with this lovely code was a few months after swapping a battery in the car. A few months later P0171 was a nemisis. Turned out my battery cables were corroded badly, pulling the boots made it obvious that the corrosion went down into the wire below the sheath of the wires. My fix ended up being replacing the wires for all battery cables.
