My best guess for the bad effiency is going to be the O2 sensor.... Usually when the MPG'* suddenly start to tumble off the sensor is slowing.... I was doing as bad a 14mpg when mine failed, and I wasn't even flooring it...
Normal is 18-21 in town for me depending on the amount of pedal pounding I do...

I'll make the assumption that you have the original sensor, and they do last about 60,000 miles roughly.... But even at that point they have lost some of their signaling ability, or basicly they slow in their signaling rate, causing the engine to run richer for longer...
when you get the new sensor install, reset the PCM( dissconnect battery ) and allow the PCM to relearn... Then the next time you go on a good highway run clock the MPG'* for us... If you stay between 70-80<---- Yes inbetween this range, you should do 29-32..

I'm gonna say O2 as a priority. It'* easier, and will probably have the most notable affect for you. Maybe if that'* the problem, then you MPG getting back to normal might help pay for the Cat....
